CALEA Assessment Plan

I am the new accreditation manger for my agency. We just enrolled in the CALEA Law Enforcement accreditation program. The tutorials said that we should have a written assessment plan. I am struggling to come up with an assessment plan. Does anyone have an example that I could review or know where I could fine a template?
